On Stardate 50979 on a shuttle surveys mission, the HoloDoc and Kim accompanied her, the shuttle was attack by a unknown species. Kim and Jetal where critically injured by the alien weapon designed to keep energy in the body, slowly frying the spinal cord and the brain. With the lack of time the Doc can only save one and he choose Harry Kim. With that decision the Doc developed a feedback loop between his ethical and cognitive subroutines. Captain Janeway had no other choice but to erase any memories of Ensign Jetal to save the Doc.
